Referral for consideration for admission to our homes is primarily coordinated with your Tri-Counties Regional Center Service Coordinator.

For Intermediate Care Facilities, regulations specify the following eligibility criteria for admission. .

ICF/DD-Habilitative

I. Must have two developmental deficits in either of these domains:


A. Self Help Domain
1. Eating
2. Toileting
3. Bladder control
4. Dressing

B. Social-Emotional Domain
1. Unacceptable social behavior
2. Aggression
3. Self-injurious behavior
4. Smearing
5. Property destruction
6. Runs or wanders away
7. Temper tantrums

Conditions which may exclude beneficiaries from placement:

1. Aggression – has had violent episodes which have caused serious physical injury in the past year.
2. Self-injurious behavior causes severe injury which requires physician attention at least once per year.
3. Smearing-smears at every opportunity
4. Has an active communicable disease that is required to be reported
5. Has a decubitus of any stage.

ICF/DD-Nursing


I. Must have two developmental deficits in any one or combination of these three domains:

A. Self Help Domain
1. Eating
2. Toileting
3. Bladder control
4. Dressing


B. Social-Emotional Domain
1. Unacceptable social behavior
2. Aggression
3. Self-injurious behavior
4. Smearing
5. Property destruction
6. Runs or wanders away
7. Temper tantrums

C. Motor Domain
1. Ambulation
2. Crawling and standing
3. W/C mobility
4. Rolling and sitting


II. Must have need for intermittent skilled nursing services such as:

1. Apnea monitoring
2. Colostomy care
3. Gastrostomy feeding and care
4. Naso-gastric feeding
5. Tracheostomy care and suctioning
6. Oxygen therapy
7. I.P.P.B.
8. Catheterization
9. Wound irrigation & dressing
10. Licensed nurse evaluation on an intermittent basis – see eligibility guidelines
11. Special feeding assistance
12. Repositioning to avoid skin breakdown which would lead to decubitus ulcers and contractures.


III. Conditions which exclude beneficiaries from placement:

1. Aggression – has had violent episodes which have caused serious physical injury in the past year.
2. Self-injurious behavior causes severe injury which requires physician attention at least once per year.
3. Smearing-smears at every opportunity
4. Has an active communicable disease that is required to be reported
5. Has a decubitus of stages III or IV.
